Overview

Long Term Care Facilities (CHSLDs) present unique opportunities and challenges to provide care for a very vulnerable population.  In this workshop, we will discuss the common challenges that physicians can face in long term care facilities.  We will review the 7 Choosing Wisely targets for care in long term care, a standardized approach to pneumonia and UTIs and the establishment of level of interventions.  We will also discuss Potentially Avoidable Transfers to the Emergency department.

Objectives

By the end of the sessions, participants should:

1. Be aware of the 7 Choosing Wisely targets for long term care

2. Have an approach to care for Pneumonia and UTI in long term care

3. Be able to obtain a level of intervention for residents in Long Term Care.

4. Understand the literature surrounding Potentially avoidable transfers to Emergency Rooms from Long term care.
